Evaluation is important to the growth, fidelity, and sustainability of the HFW process.  PA HFW workforce are responsible for implementing two evaluation tools, The Chart Forms and The WFI-EZ, throughout the process with each youth and family. 

YFTI has worked very hard to standardize the training, coaching, and credentialing of HFW and took on the task of standardizing a way to show outcomes.  This was done by standardizing chart documentation for the HFW process statewide.  By standardizing chart documentation, YFTI created a sustainable evaluation tool that is utilized by wraparound providers in diverse regions to increase fidelity and improve outcomes for families.  It is the responsibility of the HFW workforce to work together to complete the Chart Forms.  The Chart Forms were created based on the HFW Framework as well as the skills sets of each workforce member.

Responsibility of ensuring each of the tools is completed is the shared responsibility of the HFW coach and workforce.  Chart Form Training video supports workforce in learning the evaluation requirements and skills of data collection.  Chart Form training requires a knowledge assessment score of 80% or higher as well as a sample contact note submitted to YFTI for credit. This training is part of the credentialing requirements.
