Age Range

Youth Ages 10-21

Youth under 10 years of age are reviewed on a case-by-case basis.

Other Criteria

Mental Health (MH) Diagnosis and High-End Users of Mental Health Services which may include any of the following:

  • Involved with Juvenile Justice (JPO) and the Office of Children and Youth (OCY)
  • Youth At Risk for Out of Home Placement
  • Youth Transitioning Back to The Community from an Out of Home Placement
  • Families with Complex Needs Across Systems (MH, JPO, OCY, Education, Intellectual Disability, Drug & Alcohol)
