Successful transition doesn’t begin when services are ending, it begins the moment we start working alongside youth and families. This webinar will explore how Family Driven Teaming builds self-efficacy from day one by helping youth and families identify their strengths, develop transferable skills, strengthen natural supports, and take increasing ownership of their goals and decision-making. Participants will learn practical strategies for fostering confidence, self-advocacy, and independence throughout the helping process so that transition becomes a celebration of readiness rather than an ending. Join us as we discuss how intentional skill-building, family leadership, and community connections create the foundation for sustainable success long after formal services have ended.
