Program Fidelity Self-Assessment

As part of a commitment to continuous quality improvement, sites receiving level 2 or level 3 training will complete a quarterly Program Fidelity Self-Assessment to evaluate how effectively they are implementing key implementation domains of the Family Driven Teaming model. They will also provide simple quarterly tracking data around their enrollments, active families, and discharges.

This self-assessment is designed to help programs identify what healthy program implementation looks and feels like across six key domains:

  • Leadership
  • Staffing
  • Census
  • Teaming
  • Planning
  • Evaluation

Program leadership will have the ability to reflect on their current practices, identify strengths, and pinpoint areas for growth.

Plan-Do-Study-Act Cycle

What is a PDSA Cycle?

Following the completion of the self-assessment, sites will engage in Plan-Do-Study-Act (PDSA) cycles to address areas identified and prioritized as opportunities for improvement. PDSA cycles are a structured, incremental method for testing and refining changes in practice. They support teams in making data-informed decisions and building sustainable improvements over time.

Plan

Identify a specific area for improvement based on results of the self-assessment. Develop a plan to test a change, including what will be done, who will do it, when, and how success will be measured.

Do

Implement the change on a small scale.

Study

Collect and analyze data to understand the impact of the change. Compare the results to your expectations.

Act

Decide whether to adopt the change, adapt it for further testing, or adandon it. Use what you’ve learned to inform next steps.

Input and Visualize Data Using Interactive Dashboards

We offer a convenient web-based platform to assist in implementation and quality improvement. Data is entered into the YFTI dashboard by an identified individual at each site on behalf of the program. Sites are encouraged to meet and collaborate on self-assessment ratings and priority needs for the PDSA cycles. 

Data is entered directly into the Fidelity Self-Assessment Tracking and PDSA dashboards, then sites can view their progress each quarter, and can also view a summary of all of their quality improvement strategies.
