Youth Ages 5-21

Must Have Complex Emotional and Behavioral Health Issues

Have Medical Assistance

At Risk for Out of Home Placement

Multi-System Involvement

Additional Requirement for Team 2: Multi-System Involvement, including Involvement with Child Welfare and/or Juvenile Probation

Exclusions

  • Cannot have a current doctor’s recommendation for upcoming Residential Treatment Facility (RTF) placement in place.
  • If currently in an RTF, can begin formal direct care upon discharge.

Referrals with Autism Spectrum Disorder (ASD) and/or Intellectual Disabilities (ID) Diagnosis are reviewed on a case-by-case basis.

Some form of documentation must be presented with the referral form to show youth’s level of functioning in order to complete assessment of referral appropriateness (I.E. Any ASD screenings or doctor’s notes pertaining to the youth’s level of comprehension and full-scale intelligence quotient (FSIQ) scores).
